HOW DO I....
OBTAIN A REPEAT PRESCRIPTION?

If you require long-term treatment your doctor may give you your medication on a repeat prescription. Repeat prescriptions are printed on the computer which also prints out a list of all your medicines.
To obtain further repeat prescriptions either:

(i) Hand this list into the surgery allowing at least two full working days for your request to be processed.

(ii) Post the list, together with a stamped self-addressed envelope, to the surgery. Please allow at least two full working days plus posting time for your request to be processed.

(iii) Request your repeat prescription on-line. The first step is to call into the surgery and tell the receptionist that you would like to register for EMIS access. You will need to provide proof of identity for the receptionist. This may be a driving licence, passport, or a recent utility bill. She will provide you with a form with a unique PIN number (please keep this secret!). The level of security for this site is the same that you would experience when using on-line banking. Once you have your registration details you can go home and register on-line. You must allow two full working days for your request to be processed.

OBTAIN A SICKNESS CERTIFICATES?

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may, however, require you to complete a self-certification form (SC1) which is available from your employer or at our reception. For any illness lasting longer than seven days you will need to see the doctor so that a sickness certificate (FMed3) can be issued and for any subsequent renewal of the certificate.
