HOW DO I.... REGISTER WITH THE PRACTICE? If you wish to register with us simply call in at the surgery and one of our receptionists will advise you. An appointment for a simple registration medical will be offered with a practice nurse. Please bring a urine specimen and your completed questionnaire plus a list of your medication with you when you attend this appointment. |
