Patients'
Rights
You
will be treated as a partner in the care and attention you receive. You will be
treated as an individual and will be given courtesy and respect irrespective of
your ethnic origin, religious beliefs, personal attributes or the nature of your
health problems.
Following
discussion you will receive the most appropriate care given by suitably qualified
people. No care or treatment will be given without your informed consent.
You have the right to see your health records (which will be kept confidential)
subject to any limitation in the law.
We will give you full information about the services we offer. Every effort will
be made to ensure that you receive that information which directly affects your
health and the care being offered.
People involved in your care will give you their names and ensure that you know
how to contact them. Please let us know if you change your name or address.
It is our job to give you treatment and advice. In the interest of your health
it is important for you to understand all the information given to you. Please
ask us questions if you are unsure of anything.
We will provide you with information about how to make suggestions or complaints
about the care we offer. We want to improve our services and would therefore welcome
any comments you have.
Patients'
Responsibilities
Please
do everything you can to keep appointments. Tell us as soon as possible if you
cannot attend. Be ready to give us details of your past illnesses, medication,
hospital admissions and any other relevant details. Please ask for night visits
only when you feel it is truly necessary and home visits by the doctor only when
patients are too ill to visit the surgery.
We
are committed to training our staff to the highest standards. They will at all
times treat you with courtesy and respect. Please treat them with the same respect
and courtesy. We need help too. At times their job can be very difficult so please
be patient. If there is a delay it is usually because an emergency is being dealt
with as well!
If your doctor is delayed or called out to attend an emergency the receptionists
will inform you of any delay. If you are unable to wait, an appointment with another
doctor will be offered to you or, alternatively an appointment with your usual
doctor at the next available time.
Complaints
We
realise that occasionally things do not go as smoothly as we would like. If you
think this has happened to you please come and talk to your doctor or the Head of Practice so that we can sort out the problem.
Confidentiality
and Data Protection
The practice is registered under the Data Protection Act 1998 and we will keep your health records confidential at all times.
The Act covers both electronic and paper records. During the course of their work most members of staff have access to their patients' details. We have a duty to keep information about you confidential and to protect your privacy.
All members of staff have signed a Confidentiality Agreement and follow strict guidelines therein.
You have a right to see your records subject to any limitations in law. If you wish to exercise this right please ask to see the practice manager.
